The Primary Care NP/PA’s responsibilities include:

• Functions independently to perform age-appropriate history and physicals for complex, acute, critical, and chronically ill patients
• Conducts annual wellness exams
• Diagnosing and developing treatment plans for homebound patients for acute, episodic, or chronic illnesses via analyzing and interpreting medical histories, symptoms, physical findings, or diagnostic imaging.
• Maintaining standards of care and professional practice guidelines.
• Collaborating with our team of specialists to develop treatment plans for high-risk patients.
• Partnering with local home health agencies, labs, imaging companies, and vendors.
• Communicating with patients and caregivers regarding self-management of diagnoses.
• Responding to patient and caregiver questions in a timely and knowledgeable manner.
• Maintaining compliance with HIPAA and Health Care regulations.

Interested candidates should have:

• One year of home health experience preferred, but not mandatory.
• Willingness to drive from patient to patient within the specified territory.
• Active license in the state to which you are applying, new graduates are welcome.

The Business Development Coordinators Essential Functions include:

• Being an expert witness for the company in every situation. Must be able to answer questions related to the history, mission, and vision of the company as well as practice philosophy and general questions that may arise.
• Ensuring that project/department milestones/goals are progressive with the expected timeline, following the approved budget.
• Planning, implementation, and following-up with all community-related events.
• Assisting in Developing sales by marketing initial presentations to potential facilities; explaining our services and the added value to both the patient and the facility.
• Maintaining and broaden networking to benefit the practice.
• Meeting with ALFs, home health agencies, patients, and other referral sources to help establish new patients.
• Attending and advertise health fairs, conferences, etc.
• Contributing to team effort by accomplishing related tasks as needed.

A great deal of team collaboration is required in an effort to provide comprehensive patient care in this supportive role.

Having excellent time management and multi-tasking skills, strong organizational and planning skills, and the ability prioritize work are essential for this role. 

The M.A.’s responsibilities include:

• Acting as the liaison between Medical Staff, Patients, Pharmacies, and Hospitals.
• Completing Triage duties while managing patient medical records.
• Transmiting prescription refills as directed by the assigned provider.
• Verifying all patient information is accurate and review medical histories to gather additional information as needed.
• Obtaining Lab Results and documentation in the patient’s medical records.
• Collaborating with the Scheduling team while coordinating urgent and same-day patient visits.
• Responding to patient and caregiver questions in a timely and knowledgeable manner.
• Maintaining compliance with HIPAA and Health Care regulations.

Interested candidates should have:

• Medical Assistant Certification required. 
• High school diploma or equivalent; completion of a medical assistant program, preferred. 
• Primary Care experience preferred. 
• 2 years of experience as an M.A. preferred.*
• Knowledge of medical terminology, anatomy, and basic healthcare principles.

*Applicants with more than 1 year of experience, but no M.A. certification, MPS will assist the new hire in funding the exams cost. If an applicant does not have a full year of MA experience, and no certification, MPS will assist the new hire in funding the schooling and exam cost.
